Hi Bill, I never heard it in Italian. I simply thought that he was dead but kept on dreaming.

  I don't know Italian idioms very well, but there is an idiom in French that's similar: ventre a terre.  It means something like "hell for leather" in English.  I bet that's what's going on here as well.
